Wolverhampton - Community Adult social worker
We are seeking a dedicated and compassionate Community Adult Social Worker to join our team in Wolverhampton. As a key member of our Qualified Social Work department, you will be responsible for providing support and assistance to vulnerable adults in the community.
Key responsibilities and duties include:
- Conducting assessments to determine the needs of adult service users
- Developing and implementing care plans to support individuals in achieving their goals
- Providing advocacy and support to help service users access necessary resources and services
- Working closely with other healthcare professionals and agencies to ensure holistic care for service users
- Monitoring and reviewing the effectiveness of care plans and making adjustments as needed
The ideal candidate will possess the following essential qualifications and skills:
- Qualified Social Worker with relevant experience in adult social care
- Registration with Social Work England
- Strong communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Empathy, patience, and a non-judgmental approach to working with vulnerable adults
Additional requirements for this role include a valid driver's license and access to a vehicle, as the position may involve home visits and travel within the community.
