- Providing advice, advocacy, and emotional support to clients and their families, ensuring they are empowered to make informed decisions.
- Working collaboratively with multidisciplinary teams and external agencies to ensure integrated service delivery.
- Maintaining accurate and up-to-date case records and documentation in accordance with local policies and procedures.
To be successful in this role, you will need the following essential qualifications and skills:
- A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Registration with Social Work England.
- Experience working with adults in a social work capacity, preferably within a community or locality setting.
- Strong understanding of relevant legislation, policies, and best practices in adult social care.
- Excellent communication, interpersonal, and organizational skills.
