We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's SW - Hospital Team in Conwy. This role offers an exciting opportunity to work within a dynamic environment, supporting adults in a hospital setting and ensuring they receive the care and resources they need for a smooth transition back into the community. As a key member of our team, you will play a vital role in assessing and addressing the social care needs of patients, collaborating closely with healthcare professionals to deliver high-quality outcomes.
Your key responsibilities will include:
Conducting comprehensive assessments of patients' social care needs. Developing and implementing care plans in collaboration with multidisciplinary teams. Providing support and guidance to patients and their families throughout the hospital stay. Facilitating discharge planning and ensuring appropriate follow-up services are in place. Maintaining accurate records and documentation in accordance with legal and organizational standards.The ideal candidate will possess the following essential qualifications and skills:
A recognized Social Work qualification (e.g., BA/BSc in Social Work or equivalent). Current registration with Social Care Wales. Experience working in adult social care, preferably within a hospital setting. Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
