We are currently seeking a dedicated and experienced Qualified Social Worker (QSW) to join our team at a leading hospital in Shropshire. As a vital member of the healthcare team, the successful candidate will play a key role in providing support and interventions to patients and their families to ensure their well-being and safety.
Key responsibilities and duties:
- Conduct comprehensive assessments of patients' social care needs and develop care plans accordingly.
- Provide practical and emotional support to patients and their families, offering guidance on coping strategies and accessing community resources.
- Work collaboratively with other healthcare professionals, including doctors, nurses, and therapists, to ensure holistic care for patients.
- Advocate on behalf of patients to ensure their rights are upheld and that they receive the appropriate support and services.
- Maintain accurate and up-to-date records of interventions and progress, ensuring confidentiality and compliance with data protection regulations.
Essential qualifications and skills:
- Qualified Social Worker (QSW) registration with the Health and Care Professions Council (HCPC).
- Demonstrated experience in a hospital or healthcare setting, preferably within a social work capacity.
- Strong communication and interpersonal skills, with the ability to build rapport with patients and colleagues.
- Excellent assessment and decision-making abilities, with a focus on person-centered care.
- Knowledge of relevant legislation and policies relating to social work practice in healthcare.
