We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work - Physical Disabilities (PD) Team in Southwark. In this role, you will be responsible for providing high-quality social work services to adults with physical disabilities, ensuring they receive the support and resources necessary to enhance their quality of life. You will work collaboratively with individuals, families, and multidisciplinary teams to promote independence and well-being.

Your key responsibilities will include:

  • Conducting comprehensive assessments of individuals with physical disabilities to identify their needs and strengths.
  • Developing and implementing tailored support plans in collaboration with service users and their families.
  • Providing ongoing support and advocacy to individuals, ensuring their voices are heard and respected.
  • Working closely with healthcare professionals, community organizations, and other stakeholders to coordinate services and resources.
  • Maintaining accurate and up-to-date records in compliance with statutory requirements and organizational policies.

The ideal candidate will possess essential qualifications and skills, including:

  • A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Registration with Social Work England.
  • Experience working with adults with physical disabilities or similar client groups.
  • Strong assessment and analytical skills, with the ability to develop effective care plans.
  • Excellent communication and interpersonal skills, with a compassionate approach to service delivery.
