Your key responsibilities will include:
Conducting comprehensive assessments of children's needs and circumstances. Developing and implementing individualized discharge plans in collaboration with families and professionals. Providing ongoing support and guidance to families pre- and post-discharge. Maintaining accurate records and documentation in accordance with local policies and procedures. Participating in case conferences and meetings to advocate for the best outcomes for children.The ideal candidate will possess the following essential qualifications and skills:
A recognized social work qualification (e.g., BA/MA in Social Work). Current registration with Social Work England. Proven experience working within children's social services, particularly in discharge planning. Strong communication and interpersonal skills, with the ability to build rapport with children and families.
