As a member of our team, you will be responsible for conducting comprehensive assessments and reviews of service users' needs. Your key responsibilities will include:
- Developing and implementing care plans that promote independence and well-being.
- Working collaboratively with service users, families, and other professionals to ensure effective service delivery.
- Monitoring and reviewing the progress of care plans, making adjustments as necessary.
- Providing advice and support to service users and their families regarding available services and resources.
- Maintaining accurate and up-to-date records in compliance with local authority policies.
To be considered for this position, you must possess the following essential qualifications and skills:
- A relevant social work qualification (Degree in Social Work, CQSW, DipSW, or equivalent).
- Current registration with Social Work England.
- Proven experience in adult social care, particularly in promoting independence.
- Strong assessment and analytical skills.
- Excellent communication and interpersonal skills, with the ability to engage effectively with diverse populations.
