Adult's SW - Community/MH Team - London Borough of Tower Hamlets
The London Borough of Tower Hamlets is seeking a dedicated and passionate Adult Social Worker to join our Community/Mental Health Team. Key responsibilities and duties include:
- Conducting comprehensive assessments of service users' needs and risks.
- Developing and implementing person-centered care plans in collaboration with service users and their support networks.
- Providing ongoing support and advocacy to individuals experiencing mental health challenges.
- Working closely with multidisciplinary teams, including mental health professionals, to coordinate services effectively.
- Monitoring and reviewing care plans to ensure they meet evolving needs and goals.
- Maintaining accurate and up-to-date case records in accordance with local policies and procedures.
To be considered for this position, candidates must possess the following essential qualifications and skills:
- A recognized social work qualification (e.g., Degree in Social Work, CQSW, DipSW).
- Registered with Social Work England (SWE).
- Proven experience in adult social work, particularly within a community or mental health setting.
- Strong assessment and analytical skills, with the ability to make informed decisions.
- Excellent communication and interpersonal skills, with a commitment to working in partnership with service users.
- Knowledge of relevant legislation, policies, and best practices in adult social care and mental health.
