Are you a dedicated and compassionate social worker looking to make a meaningful impact in the lives of adults? Barnet's Locality Team is seeking a Qualified Social Worker to join our dynamic team. In this role, you will work collaboratively with individuals and their families to assess needs, develop care plans, and provide support to enhance their well-being and independence within the community.
Your key responsibilities will include:
Conducting comprehensive assessments of individuals' needs and risks. Developing and implementing person-centered support plans. Collaborating with multi-disciplinary teams and external agencies to ensure effective service delivery. Providing ongoing support and monitoring of individuals' progress. Maintaining accurate and timely documentation in line with legislative requirements.To be considered for this role, you must possess essential qualifications and skills, including:
A recognized social work qualification (e.g., BA/MA in Social Work). Registration with Social Work England. Experience in adult social work, particularly within a locality or community setting. Strong assessment and analytical skills.
