We are seeking a qualified social worker to help individuals navigate their challenges and improve their quality of life through effective interventions and support services.
As a member of our team, your key responsibilities will include:
Conducting comprehensive assessments of individuals' needs and circumstances. Developing and implementing tailored care plans in collaboration with clients and their families. Providing ongoing support and monitoring of clients’ progress. Collaborating with multidisciplinary teams and external agencies to coordinate services. Maintaining accurate and up-to-date case records in accordance with legal and organizational standards.The ideal candidate will possess the following essential qualifications and skills:
A recognized social work qualification (e.g., CQSW, DipSW, or equivalent). Registered with Social Work England. Proven experience in adult social work, preferably within a community setting. Strong assessment and analytical skills, with the ability to identify and address complex needs.
