Join our dedicated Transitions Team in Rotherham as an Adult Social Worker (SW) and make a meaningful impact in the lives of vulnerable adults. We are seeking a passionate and skilled individual to support clients transitioning into adulthood, ensuring they receive the necessary resources and guidance to thrive. You will work collaboratively with multidisciplinary teams, families, and external agencies to promote independence and well-being among service users.

Your key responsibilities will include:

Conducting comprehensive assessments to identify the needs of individuals transitioning into adulthood. Developing, implementing, and reviewing person-centered care plans in partnership with clients and their families. Facilitating access to appropriate services and support systems, including housing, education, and employment opportunities. Advocating for clients' rights and ensuring compliance with statutory requirements and best practices. Providing guidance and support to families, helping them navigate the transition process.

The ideal candidate will possess essential qualifications and skills, including:

A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Experience working with adults, particularly in transition settings. Strong communication and interpersonal skills, with the ability to build rapport with clients and stakeholders. Knowledge of relevant legislation and policies affecting adult social care.
