Adult's SW - Hospital Team - West Sussex
An experienced Adult Social Worker is required to join a busy hospital team, supporting adults with a range of care and support needs to achieve safe, timely and person-centred outcomes. This is an opportunity to make a meaningful contribution to hospital discharge planning while ensuring individuals are fully involved in decisions about their care, independence and wellbeing.
You will manage a varied caseload, completing strengths-based assessments, reviews and safeguarding enquiries in line with relevant legislation, policies and professional standards. Working closely with individuals, families and carers, you will identify eligible needs, develop proportionate support plans and arrange appropriate services, including reablement, community support, residential or nursing care where required.
The role involves close partnership working with health professionals, occupational therapists, housing services, care providers and other agencies. You will contribute effectively to multi-disciplinary meetings, provide clear professional advice and help resolve complex situations, including those involving risk, capacity, self-neglect or disputes regarding care arrangements. Accurate recording, sound professional judgement and effective time management will be essential in this fast-paced environment.
Applicants must hold a recognised Social Work qualification and current registration with Social Work England. You should have recent experience of working with adults in a statutory social care, hospital or discharge setting, together with a good understanding of the Care Act, Mental Capacity Act, safeguarding procedures and relevant duties relating to carers. Experience of managing complex cases and arranging services within available resources is highly desirable.
You will be a confident communicator who can build constructive relationships with people from diverse backgrounds. A flexible, resilient and collaborative approach is essential, as is the ability to prioritise competing demands while maintaining high professional standards. A valid driving licence and access to transport may be required, depending on service needs.
