Your key responsibilities will include:

  • Conducting comprehensive assessments of needs and risks to inform care planning.
  • Developing, implementing, and reviewing care plans in collaboration with service users and other professionals.
  • Providing direct support and intervention to adults in need, including advocacy and safeguarding.
  • Maintaining accurate and timely records in accordance with local policies and procedures.
  • Working collaboratively with multidisciplinary teams and external agencies to ensure holistic support.

To be successful in this role, you will need to possess the following qualifications and skills:

  • A recognized social work qualification (e.g., BA in Social Work, MA in Social Work, or equivalent).
  • Registration with Social Work England.
  • Experience working in adult social care or a related field.
  • Strong assessment and analytical skills, with the ability to make sound judgments.
  • Excellent communication and interpersonal skills, with a compassionate approach to service users.
