As a Social Worker in the Adults Community team, your key responsibilities will include:
Conducting comprehensive assessments to identify the needs and strengths of clients. Developing, implementing, and reviewing care plans in collaboration with clients and their families. Providing emotional and practical support to clients, helping them navigate complex systems. Working closely with healthcare providers, community organizations, and other stakeholders to ensure coordinated care. Advocating for clients’ rights and access to services, ensuring their voices are heard.The ideal candidate will possess the following essential qualifications and skills:
A degree in Social Work and registration with Social Work England. Proven experience in adult social work, preferably within a community setting. Strong assessment and analytical skills, with the ability to create tailored care plans. Excellent communication and interpersonal skills, with a compassionate approach to client interactions.
