Adult's SW - Locality Team - Oxfordshire

Oxfordshire County Council is seeking a dedicated and compassionate Qualified Social Worker to join our Adult's SW - Locality Team. This role is pivotal in providing high-quality support to adults in our community, ensuring their needs are met and their voices are heard. As a member of our team, you will work collaboratively with service users, families, and other professionals to deliver effective interventions that promote independence and well-being.

Key responsibilities include:

  • Conducting comprehensive assessments to identify the needs and strengths of service users.
  • Developing, implementing, and reviewing care plans in partnership with individuals and their families.
  • Providing ongoing support and guidance to service users, ensuring they have access to appropriate resources and services.
  • Collaborating with multidisciplinary teams, including health professionals, to ensure holistic care delivery.
  • Maintaining accurate and timely records in accordance with local authority policies and procedures.

The ideal candidate will possess the following essential qualifications and skills:

  • A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Registration with Social Work England.
  • Demonstrable experience in adult social work, particularly within a locality team setting.
  • Excellent communication, interpersonal, and analytical skills.
  • Ability to work effectively under pressure and manage a varied caseload.
