We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work Community Team in Doncaster. This role is essential in providing high-quality support and services to vulnerable adults within our community. You will be responsible for assessing needs, developing care plans, and working collaboratively with other professionals to ensure the best outcomes for those we serve.

Key responsibilities include:

  • Conducting comprehensive assessments of clients' needs and circumstances.
  • Developing and implementing tailored care plans in collaboration with clients and their families.
  • Providing ongoing support and monitoring of client progress, adjusting care plans as necessary.
  • Working closely with multidisciplinary teams, including healthcare providers, to coordinate services.
  • Maintaining accurate and up-to-date case records in compliance with local policies and procedures.

To be considered for this role, you must possess the following essential qualifications and skills:

  • A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Current registration with Social Work England.
  • Experience in adult social care, particularly within community settings.
  • Strong communication and interpersonal skills, with the ability to build rapport with clients and their families.
  • Excellent organizational skills and the ability to manage a varied caseload effectively.
