As a key member of the Assessment Team, you will be responsible for:

Conducting comprehensive assessments of adults in need of support, ensuring their voices are heard and their needs are prioritized. Developing and implementing person-centered support plans in collaboration with individuals, families, and other professionals. Providing ongoing support and guidance to clients, ensuring they have access to necessary resources and services. Working closely with multidisciplinary teams and external agencies to promote integrated care and safeguarding practices. Maintaining accurate and up-to-date case records in compliance with local and national standards.

The ideal candidate will possess the following essential qualifications and skills:

A relevant social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Current Social Work England registration. Proven experience in adult social work, particularly in assessments and support planning. Strong communication and interpersonal skills, with the ability to build rapport with clients and stakeholders. Knowledge of relevant legislation and best practice in adult social care.
