We are seeking a dedicated and experienced Senior Social Worker to join our Preparing for Adulthood team in Dudley. This pivotal role focuses on supporting young individuals with additional needs as they transition into adulthood. You will work closely with families, educational institutions, and community organizations to ensure a seamless transition that promotes independence and well-being for our clients.

Key responsibilities include:

Conducting comprehensive assessments to identify the needs and aspirations of young people with disabilities. Developing and implementing personalized support plans that facilitate a successful transition to adulthood. Collaborating with multi-disciplinary teams to coordinate services and resources for clients. Providing guidance to families and caregivers, empowering them to support their loved ones effectively. Monitoring progress and adjusting support plans as necessary to meet changing needs.

The ideal candidate will possess the following essential qualifications and skills:

A recognized social work qualification (e.g., BA/MA in Social Work). Current registration with Social Work England. Significant experience in social work, particularly with young people with disabilities. Strong knowledge of legislation and best practices related to preparing for adulthood. Excellent communication and interpersonal skills, with the ability to build rapport with clients and stakeholders.
