In this role, you will work collaboratively with families, children, and other professionals to provide high-quality support and intervention to children with disabilities and their families.
Your key responsibilities will include:
- Conducting assessments and developing care plans tailored to the individual needs of children with disabilities.
- Working closely with families to provide guidance, support, and practical solutions to challenges they face.
- Collaborating with multi-disciplinary teams, including educators, health professionals, and other social workers, to ensure comprehensive care.
- Monitoring and reviewing the progress of care plans, making adjustments as necessary to meet changing needs.
- Maintaining accurate and up-to-date case records in compliance with local authority policies and procedures.
The ideal candidate will possess essential qualifications and skills, including:
- A recognized social work qualification (e.g., BA/MA in Social Work or equivalent).
- Registration with Social Work England.
- Experience working with children with disabilities and an understanding of relevant legislation and frameworks.
- Strong communication and interpersonal skills, with the ability to engage effectively with children, families, and professionals.
- Excellent organizational skills and the ability to manage a caseload effectively.
