Are you a dedicated and passionate Social Worker looking to make a meaningful impact in the lives of adults? Join our Transformation Team in Salford, where you will play a crucial role in supporting individuals to achieve their desired outcomes. We are seeking a qualified social worker to help drive positive change and enhance the quality of life for those in our care.
As a member of the Transformation Team, your key responsibilities will include:
Conducting comprehensive assessments to identify the needs and strengths of individuals. Developing and implementing tailored support plans in collaboration with service users and their families. Facilitating access to services and resources, ensuring individuals receive the support they need. Advocating for the rights and interests of service users within the community. Working closely with multidisciplinary teams to promote integrated service delivery. Monitoring and reviewing progress, making adjustments to support plans as necessary.The ideal candidate will possess the following essential qualifications and skills:
A recognized social work qualification (e.g., BA/MA in Social Work). Registration with Social Work England. Experience working within adult social care, particularly in transformation or change management. Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
