As a Social Worker within the Leaving Care Team, your key responsibilities will include:

  • Conducting assessments and developing tailored care plans for young people.
  • Providing emotional and practical support to help young people achieve their goals.
  • Working collaboratively with other agencies and stakeholders to ensure comprehensive support.
  • Monitoring and reviewing the progress of care plans to ensure effective outcomes.

The ideal candidate will possess the following essential qualifications and skills:

  • A recognized Social Work qualification (BA/MA Social Work or equivalent).
  • Registration with Social Work England.
  • Experience working with young people, particularly in a leaving care or similar setting.
  • Excellent communication and interpersonal skills.
  • Strong organizational skills and the ability to manage a varied caseload.
