We are seeking an experienced Qualified Social Worker to join the Transition Team in Barnet on a temporary basis, offering a competitive rate of £32 - £34 per hour (Umbrella). This role is pivotal in supporting young individuals as they transition from children's services to adult services, ensuring they receive the necessary support and guidance during this critical period in their lives.

Key responsibilities include:

  • Conducting assessments of young people's needs and developing tailored support plans.
  • Collaborating with families, educational institutions, and other professionals to facilitate smooth transitions.
  • Providing ongoing case management and support to young individuals, ensuring their voices are heard.
  • Monitoring progress and adjusting support plans as necessary to meet changing needs.
  • Keeping accurate records and documentation in line with local authority policies and procedures.

To be considered for this role, candidates must possess the following qualifications and skills:

  • A recognized social work qualification (e.g., CQSW, DipSW, or equivalent).
  • Current registration with Social Work England.
  • Proven experience in a social work setting, particularly within transition services.
  • Strong communication and interpersonal skills, with the ability to engage effectively with young people and their families.
  • Excellent organizational skills and the ability to manage a caseload effectively.
