We are currently seeking a dedicated and compassionate Qualified Social Worker to join our team in Southend for the AGEING WELL WEST COMMUNITY project. As a key member of the team, you will play a crucial role in supporting and empowering older adults to live fulfilling and independent lives.

Key responsibilities and duties:

  • Conduct comprehensive assessments of the needs and strengths of older adults in the community.
  • Develop and implement personalized care plans to support ageing well and promote independence.
  • Provide advocacy and support to clients in accessing appropriate services and resources.
  • Collaborate with multidisciplinary teams and external agencies to ensure holistic care for older adults.
  • Maintain accurate and up-to-date records of client interactions and progress.

Essential qualifications and skills:

  • Qualified Social Worker with a recognized social work qualification.
  • Registration with the Health and Care Professions Council (HCPC).
  • Demonstrated experience working with older adults and a strong understanding of issues related to ageing.
  • Excellent communication and interpersonal skills to build rapport with clients and stakeholders.
  • Ability to work autonomously and as part of a team in a fast-paced environment.
