We are currently seeking a dedicated and experienced Social Worker to join our Mental Health Hospital Team in Leicestershire. This rewarding position offers a competitive rate of £34 per hour and the opportunity to make a real difference in the lives of individuals in need of mental health support.
Key responsibilities and duties for this role include:
- Assessing the social care needs of patients within the mental health hospital setting
- Developing and implementing care plans in collaboration with multidisciplinary teams
- Providing support and advocacy for patients and their families to ensure their well-being and rights are upheld
- Participating in safeguarding procedures and risk assessments
- Engaging with community resources and services to facilitate successful discharge and aftercare plans
- Qualified Social Worker with valid HCPC registration
- Experience working within a mental health or hospital setting

- Ability to work effectively as part of a team and independently
