We are currently seeking a dedicated and experienced Qualified Social Worker to join our Hospital Discharge to Assess Team in Manchester. As a vital member of our team, you will play a crucial role in facilitating safe and timely discharges for patients, ensuring they receive appropriate support and care post-hospitalization.

Key responsibilities and duties:

  • Conduct assessments to determine the needs of patients being discharged from the hospital.
  • Develop and implement care plans in collaboration with the multidisciplinary team to support patients' transition back to the community.
  • Coordinate services and resources to meet the identified needs of patients, including arranging for home care, equipment, and other support services.
  • Advocate for patients to ensure they receive the necessary support and services to promote their well-being and independence.
  • Provide emotional support and guidance to patients and their families during the discharge process.

Essential qualifications and skills:

  • Qualified Social Worker with a valid registration with Social Work England.
  • Experience working in a hospital setting and knowledge of the discharge process.
  • Strong assessment and care planning skills with the ability to work collaboratively with other professionals.
  • Excellent communication and interpersonal skills to engage with patients, families, and colleagues effectively.
  • Ability to work under pressure, prioritize tasks, and meet deadlines in a fast-paced environment.
