We are seeking a dedicated and compassionate Qualified Social Worker to join our Assessment Team in Wembley, focusing on adult services. In this role, you will play a crucial part in safeguarding vulnerable adults and promoting their well-being. Key responsibilities include:

  • Conducting thorough assessments of adults in need of support, identifying their strengths and areas for development.
  • Collaborating with multi-disciplinary teams to create and implement effective care plans tailored to individual needs.
  • Providing ongoing support and guidance to clients and their families, ensuring they understand available services and options.
  • Maintaining accurate and up-to-date records in accordance with local policies and procedures.
  • Participating in team meetings and contributing to the development of best practices within the Assessment Team.

The ideal candidate will possess the following qualifications and skills:

  • A recognized social work qualification (e.g., Degree in Social Work or equivalent).
  • Registration with Social Work England (or equivalent regulatory body).
  • Proven experience in adult social work, particularly within assessment and care planning.
  • Strong communication and interpersonal skills, with the ability to build rapport with clients and stakeholders.
  • Excellent organizational skills and the ability to manage a caseload effectively.
