We are seeking a dedicated and compassionate Qualified Social Worker to join our Assessment Team in Wembley, focusing on adult services. In this role, you will play a crucial part in safeguarding vulnerable adults and promoting their well-being. Your expertise will be essential in conducting comprehensive assessments, developing care plans, and ensuring that individuals receive the support they need to enhance their quality of life.
Key responsibilities include:
Conducting thorough assessments of adults in need of support, identifying their strengths and areas for development. Collaborating with multi-disciplinary teams to create and implement effective care plans tailored to individual needs. Providing ongoing support and guidance to clients and their families, ensuring they understand available services and options. Maintaining accurate and up-to-date records in accordance with local policies and procedures. Participating in team meetings and contributing to the development of best practices within the Assessment Team.The ideal candidate will possess the following qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work or equivalent). Registration with Social Work England (or equivalent regulatory body). Proven experience in adult social work, particularly within assessment and care planning. Strong communication and interpersonal skills, with the ability to build rapport with clients and stakeholders.
