Derby City is seeking a qualified Social Worker specializing in Learning Disability and Transitions to join our dynamic team. This role offers an exciting opportunity to support individuals and their families as they navigate vital transitions in their lives, ensuring they receive the care and support they deserve.
As a Social Worker in this role, your key responsibilities will include:
Conducting assessments to identify the needs and strengths of individuals with learning disabilities. Developing and implementing personalized care plans in collaboration with service users and their families. Facilitating smooth transitions for individuals moving from children’s to adult services. Providing ongoing support and guidance to families, ensuring they are informed and engaged in the process. Collaborating with multidisciplinary teams and external agencies to promote holistic care and support.The ideal candidate will possess the following essential qualifications and skills:
A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Current Social Work registration with Social Work England. Experience working with individuals with learning disabilities and knowledge of relevant legislation. Strong communication and interpersonal skills, with the ability to build rapport with diverse populations.
