Your key responsibilities will include:

  • Leading assessments and reviews for individuals eligible for Continuing Healthcare funding.
  • Developing and implementing person-centered care plans in collaboration with clients and their families.
  • Providing expert guidance and support to junior social workers and other team members.
  • Ensuring compliance with relevant legislation and best practices in social work.
  • Engaging with external agencies and stakeholders to promote integrated care solutions.

The ideal candidate will possess the following qualifications and skills:

  • Registered Social Worker with a valid Social Work England registration.
  • A relevant degree in Social Work or equivalent qualification.
  • Proven experience in adult social care, particularly in Continuing Healthcare.
  • Excellent communication and interpersonal skills.
  • Strong leadership abilities, with a focus on team development and collaboration.
