Join our dynamic team in Southend-on-Sea as a CHC Lead Social Worker for Adults, where you will play a pivotal role in delivering high-quality social care services. This 12-month position offers an exciting opportunity to lead and support a dedicated team while ensuring that the needs of vulnerable adults are met with compassion and professionalism. You will be responsible for conducting assessments, developing care plans, and collaborating with multidisciplinary teams to facilitate the best outcomes for clients.
Your key responsibilities will include:
Leading assessments and reviews for individuals eligible for Continuing Healthcare funding. Developing and implementing person-centered care plans in collaboration with clients and their families. Providing expert guidance and support to junior social workers and other team members. Ensuring compliance with relevant legislation and best practices in social work. Engaging with external agencies and stakeholders to promote integrated care solutions.The ideal candidate will possess the following qualifications and skills:
Registered Social Worker with a valid Social Work England registration. A relevant degree in Social Work or equivalent qualification. Proven experience in adult social care, particularly in Continuing Healthcare. Excellent communication and interpersonal skills.
