We are seeking a dedicated and compassionate Social Worker to join our Assessment and Care Delivery team in Liverpool. This pivotal role requires an individual who is committed to improving the lives of vulnerable individuals and families within our community. As a Social Worker, you will conduct assessments, develop care plans, and provide ongoing support to clients, ensuring that their needs are met with empathy and professionalism.
Key responsibilities and duties include:
Conducting comprehensive assessments of clients’ needs and circumstances. Developing individualized care plans in collaboration with clients and their families. Providing direct support and intervention to clients, advocating for their rights and needs. Coordinating with healthcare providers, community organizations, and other stakeholders to facilitate resources and services. Maintaining accurate and timely documentation of client interactions and progress. Participating in regular team meetings and case reviews to enhance service delivery.Essential qualifications and skills required for this role include:
A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Proven experience in assessment and care delivery within a social work setting. Strong interpersonal and communication skills, with the ability to build rapport with diverse clients. Excellent organizational skills and the ability to manage a caseload effectively.
