We are seeking a dedicated and dynamic Qualified Social Worker to join our Adult's Social Work Hospital Team in West Sussex. This role is pivotal in providing high-quality social care to adults within a hospital setting, ensuring that individuals receive the support they need during critical times. You will work collaboratively with healthcare professionals, service users, and their families to promote well-being and facilitate appropriate discharge planning.

Key responsibilities and duties include:

Conducting assessments and developing care plans that address the individual needs of adults in the hospital. Providing emotional and practical support to patients and their families, ensuring their voices are heard in the decision-making process. Collaborating with multidisciplinary teams to ensure seamless transitions of care from hospital to home or other settings. Advocating for the rights and welfare of service users, ensuring compliance with relevant legislation and best practice guidelines. Maintaining accurate and up-to-date case records, adhering to confidentiality and data protection policies.

The ideal candidate will possess the following essential qualifications and skills:

A recognized social work qualification (e.g., Degree in Social Work or equivalent). Current Social Work England registration. Experience working in adult social care, particularly within a hospital environment. Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals. Excellent organizational skills and the ability to manage a varied caseload effectively.

We are seeking a dedicated and experienced Qualified Social Worker to join our Adult's SW - Hospital Team in West Sussex. This vital role involves working collaboratively within a multidisciplinary team to support adults facing complex health and social care needs. You will be responsible for assessing, planning, and implementing effective interventions to promote the well-being and independence of service users, ensuring they receive high-quality care tailored to their individual circumstances.

Your key responsibilities will include:

Conducting comprehensive assessments of service users' needs and risks. Developing and implementing care plans in partnership with service users, families, and other professionals. Providing support and guidance to individuals and their families throughout the discharge process.
