We are seeking a dedicated and passionate Qualified Social Worker to join our Adult's SW - Assessment Team in Liverpool. This role is pivotal in ensuring the safety and well-being of vulnerable adults within our community. The successful candidate will be responsible for conducting comprehensive assessments, developing care plans, and implementing interventions that promote independence and enhance quality of life for service users.

Key responsibilities include:

  • Conducting thorough assessments of individuals' needs and risks.
  • Developing and reviewing care plans in collaboration with service users and their families.
  • Working closely with multidisciplinary teams and external agencies to ensure effective service delivery.
  • Maintaining accurate and up-to-date case records and documentation.
  • Providing support and guidance to service users, advocating for their rights and needs.

The ideal candidate will possess the following essential qualifications and skills:

  • A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Registration with Social Work England.
  • Experience in adult social work, particularly in assessment and care planning.
  • Strong communication and interpersonal skills, with the ability to build rapport with clients.
  • Excellent organizational and time management skills.
