We are seeking a dedicated and compassionate Adult's Social Worker to join our Hospital Team in Bury. This role is crucial in providing high-quality social work services to adults within a hospital setting, ensuring that patients receive the support they need during their recovery process. You will work collaboratively with healthcare professionals to assess patients' social needs, develop care plans, and facilitate access to community resources.

Your key responsibilities will include:

  • Conducting comprehensive assessments of patients' social, emotional, and practical needs.
  • Developing and implementing individualized care plans in collaboration with multidisciplinary teams.
  • Providing emotional support and guidance to patients and their families throughout the hospital stay.
  • Coordinating discharge planning and connecting patients with appropriate community services.
  • Maintaining accurate and up-to-date case records and documentation in accordance with legal and organizational standards.

To be considered for this position, you must possess the following qualifications and skills:

  • A recognized Social Work qualification (e.g., BA/MA in Social Work).
  • Registration with Social Work England.
  • Experience working within a hospital or healthcare setting, preferably with adults.
  • Strong assessment and analytical skills, with the ability to make sound judgments.
  • Excellent communication and interpersonal skills, with a commitment to working collaboratively.
