We are seeking a passionate and dedicated Children's Assistant Practitioner (AP) to join our Multi-Agency Children’s Hub (MACH) Team in Middlesbrough. This is an exciting opportunity for individuals looking to make a significant impact on the lives of children and families within our community. As a key member of our team, you will work collaboratively with social workers, health professionals, and other agencies to provide high-quality support and intervention for children in need.
Your main responsibilities will include:
Assisting in the assessment and planning of support for children and families. Providing direct support to children and their families through home visits and community engagement. Collaborating with multi-agency partners to ensure comprehensive service delivery. Contributing to case reviews and team meetings, sharing insights and observations. Documenting and maintaining accurate records in line with data protection and confidentiality policies.To be successful in this role, you will need the following qualifications and skills:
A relevant qualification in social care, health, or education (e.g., NVQ Level 3 in Children and Young People’s Workforce). Experience working with children and families, preferably in a social work or educational setting. Strong communication and interpersonal skills, with the ability to build rapport with children and families. Excellent organizational skills and the ability to manage multiple cases effectively.
