Your key responsibilities will include:
- Conducting assessments of patients' social care needs in a hospital environment.
- Collaborating with healthcare professionals, patients, and families to develop comprehensive discharge plans.
- Providing information and support to patients and their families regarding available services and resources.
- Ensuring compliance with relevant legislation and best practices in social work.
- Maintaining accurate and up-to-date case records and documentation.
To be successful in this role, you must possess the following essential qualifications and skills:
- A recognized Social Work qualification (Degree in Social Work or equivalent).
- Current registration with Social Work England.
- Proven experience in hospital discharge or community social work.
