We are seeking a dedicated and compassionate Qualified Social Worker to join our Discharge To Assess Team in Bristol. The ideal candidate will possess the following essential qualifications and skills:

  • A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Registered with Social Work England.
  • Proven experience in adult social work, particularly in discharge planning and assessment.
  • Excellent communication and interpersonal skills, with the ability to build rapport with diverse individuals.
  • Strong organizational skills and the ability to manage a caseload effectively.
