  • Providing ongoing support, guidance, and advocacy to clients in accessing services and resources.
  • Working closely with multidisciplinary teams to ensure holistic and coordinated care.
  • Monitoring and reviewing care plans to ensure they remain relevant and effective in meeting client needs.
  • Documenting casework and maintaining accurate records in compliance with local policies and procedures.

The ideal candidate will possess the following essential qualifications and skills:

  • A recognized Social Work qualification (e.g., BA/MA in Social Work).
  • Registered with Social Work England.
  • Experience working with individuals with disabilities and learning difficulties.
  • Strong communication and interpersonal skills, with the ability to build rapport with clients and families.
  • Excellent problem-solving abilities and a commitment to person-centered practice.
