Adult's SW - Assessment Team - Newcastle

We are currently seeking a dedicated and experienced Adult's Social Worker to join our Assessment Team in Newcastle. As a key member of the team, you will be responsible for conducting comprehensive assessments of individuals in need of support and developing care plans to meet their specific needs.

Key responsibilities and duties include:

  • Conducting in-depth assessments to determine the level of care required for vulnerable adults
  • Working collaboratively with service users, their families, and other professionals to develop personalized care plans
  • Providing support and guidance to service users to empower them to make informed decisions about their care
  • Monitoring and reviewing care plans regularly to ensure they are effective and meeting the needs of the service users
  • Participating in multi-disciplinary meetings and case conferences to share information and coordinate care services

The ideal candidate will possess the following essential qualifications and skills:

  • Qualified Social Worker with a recognized social work qualification
  • Registered with the Health and Care Professions Council (HCPC)
  • Demonstrated experience in conducting assessments and developing care plans for vulnerable adults
  • Strong communication and interpersonal skills to engage effectively with service users and stakeholders
  • Ability to work autonomously and as part of a team in a fast-paced environment
