We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's SW - Hospital Team in Sefton. This role is essential in providing high-quality support and intervention for adults in a hospital setting, ensuring their needs are met effectively and efficiently. Key responsibilities include:

  • Providing emotional support and guidance to patients and their families throughout the hospital stay.
  • Facilitating discharge planning and connecting patients with appropriate community resources.
  • Maintaining accurate and timely documentation in compliance with statutory requirements and organizational policies.
  • Participating in regular team meetings and case reviews to enhance service delivery.

To be considered for this role, candidates must possess the following essential qualifications and skills:

  • A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Current registration with Social Work England.
  • Proven experience in adult social work, preferably within a hospital or healthcare setting.
  • Strong assessment and analytical skills, with the ability to make informed decisions under pressure.
  • Excellent communication and interpersonal skills, with a compassionate approach to service users.
