Adult's SW - Hospital Team - Sefton
We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's SW - Hospital Team in Sefton. This role is essential in providing high-quality support and intervention for adults in a hospital setting, ensuring their needs are met effectively and efficiently. As part of a collaborative team, you will work closely with healthcare professionals, patients, and their families to facilitate discharge planning, assess needs, and implement care plans that promote independence and well-being.
Key responsibilities include:
- Conducting comprehensive assessments of patients' social care needs and risks.
- Developing and implementing person-centered care plans in collaboration with multi-disciplinary teams.
- Providing emotional support and guidance to patients and their families throughout the hospital stay.
- Facilitating discharge planning and connecting patients with appropriate community resources.
- Maintaining accurate and timely documentation in compliance with statutory requirements and organizational policies.
- Participating in regular team meetings and case reviews to enhance service delivery.
To be considered for this role, candidates must possess the following essential qualifications and skills:
- A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Current registration with Social Work England.
- Proven experience in adult social work, preferably within a hospital or healthcare setting.
- Strong assessment and analytical skills, with the ability to make informed decisions under pressure.
- Excellent communication and interpersonal skills, with a compassionate approach to service users.
- A sound understanding of relevant legislation, policies, and best practices in social work.
