Join our dynamic Transition Team in Warrington as an Adult's Social Worker, where you will play a pivotal role in supporting individuals during critical life transitions. This position offers the opportunity to work collaboratively with a dedicated team to enhance the well-being and independence of adults in need of social care services. You will engage with clients, assess their needs, and develop tailored support plans that empower them to achieve their goals.
Your key responsibilities will include:
Conducting comprehensive assessments of individuals' needs and circumstances. Developing and implementing person-centered support plans in collaboration with clients and their families. Facilitating access to community resources and services to promote independence. Maintaining accurate and timely case records and documentation. Working closely with multi-disciplinary teams to ensure holistic support for clients.To be successful in this role, you must possess the following essential qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Proven experience in adult social work, particularly in transition planning. Strong communication and interpersonal skills, with the ability to build rapport with clients.
