Join our dedicated Locality Team in Worcestershire as an Adult's Social Worker, where you will play a crucial role in improving the lives of adults in our community. In this position, you will work collaboratively with individuals, families, and other professionals to assess needs, develop care plans, and provide support in a variety of settings. Your expertise will help empower vulnerable adults to live independently and safely while ensuring their rights and dignity are upheld.
Key responsibilities include:
Conducting comprehensive assessments of adults' needs and risks. Developing, implementing, and reviewing care plans in partnership with service users. Providing direct support and intervention to individuals and their families. Collaborating with multidisciplinary teams and external agencies to coordinate services. Maintaining accurate and up-to-date records in compliance with local and national guidelines. Participating in training and professional development to enhance skills and knowledge.To be successful in this role, you will need to possess the following essential qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Experience in adult social work, particularly within a locality team setting. Strong assessment and analytical skills, with the ability to make informed decisions. Excellent communication and interpersonal skills, with a compassionate approach to service users.
