The Hull Social Worker - RAST (Annual Reviews) Team is seeking a qualified social worker to join our dynamic team. In this role, you will be responsible for conducting annual reviews for individuals receiving support, ensuring their needs are met and their rights are upheld. Your work will directly contribute to the enhancement of quality of life for vulnerable populations in our community.
Key responsibilities include:
Conducting comprehensive assessments and annual reviews for service users. Developing and implementing care plans in collaboration with service users and their families. Monitoring and evaluating the effectiveness of services provided to ensure optimal support. Providing advice, support, and guidance to individuals, families, and other professionals. Maintaining accurate and timely records in compliance with legislation and policies.To be successful in this role, you will need the following essential qualifications and skills:
A recognized social work qualification (e.g., BA/MA in Social Work). Registration with Social Work England. Experience in adult social care or similar settings. Strong communication and interpersonal skills.
