We are seeking a dedicated and compassionate Hull - Social Worker to join our Hospital Team. This role is vital in providing high-quality social work services to patients and their families, ensuring they receive the necessary support during their hospital stay. The ideal candidate will work collaboratively with healthcare professionals to assess the social, emotional, and practical needs of patients, facilitating discharge planning and connecting families with community resources.

Key responsibilities include:

Conducting comprehensive assessments of patients' social and emotional needs. Developing and implementing care plans in collaboration with multidisciplinary teams. Providing counseling and support to patients and their families. Facilitating communication between patients, families, and healthcare providers. Connecting patients with appropriate community resources and services upon discharge. Documenting social work interventions and maintaining accurate patient records.

To be considered for this position, candidates must possess the following essential qualifications and skills:

A recognized social work qualification (e.g., Degree in Social Work). Registered with Social Work England. Proven experience in a hospital or healthcare setting. Strong assessment and intervention skills. Excellent communication and interpersonal abilities. Ability to work effectively in a fast-paced environment.
