We are seeking a qualified Social Worker for our Young Adults Transitions team in Bristol. In this role, you will support young individuals as they transition from adolescence to adulthood, ensuring they receive the guidance and resources necessary for a successful transition into independent living.
Your key responsibilities will include:
Conducting assessments to identify the needs and strengths of young adults. Developing and implementing individualized care plans that promote independence and wellbeing. Collaborating with families, educational institutions, and community resources to facilitate support networks. Monitoring progress and adapting plans as necessary to ensure positive outcomes. Providing advocacy and support during critical transition periods, including housing, education, and employment.To be considered for this position, you must possess the following qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registered with Social Work England. Experience working with young adults in a social work setting. Strong communication and interpersonal skills, with the ability to build rapport and trust.
