Are you passionate about making a difference in the lives of adults with learning disabilities and autism? Telford & Wrekin Council is seeking a dedicated Adult Practitioner to join our dynamic Adults with Learning Disabilities and Autism Team. In this vital role, you will work collaboratively to provide high-quality support and care, ensuring that individuals can lead fulfilling lives within their community.
Your key responsibilities will include:
Conducting comprehensive assessments of individuals’ needs and developing personalized support plans. Providing direct support and guidance to individuals and their families, promoting independence and social inclusion. Collaborating with multidisciplinary teams and external agencies to coordinate effective services. Monitoring and reviewing care plans to ensure the highest standards of care are maintained. Advocating for the rights and needs of individuals with learning disabilities and autism.To be successful in this role, you will need the following essential qualifications and skills:
A relevant social care qualification (e.g., Degree in Social Work, Diploma in Social Work, or equivalent). Experience working with adults with learning disabilities and autism. Strong communication and interpersonal skills, with the ability to build rapport with clients and their families. Knowledge of relevant legislation and best practices in social care.
