In this pivotal role, you will be responsible for conducting comprehensive reviews of care plans and assessments, ensuring that the needs of vulnerable individuals and families are met in accordance with statutory requirements and best practice guidelines.
Your key responsibilities will include:
- Carrying out regular reviews of care plans and service provision to ensure they meet individual needs.
- Collaborating with multidisciplinary teams to assess and respond to changing circumstances.
- Providing expert advice and support to colleagues and stakeholders.
- Maintaining accurate and up-to-date records in compliance with local authority policies.
- Participating in case conferences and contributing to service development initiatives.
To be considered for this role, you must possess the following qualifications and skills:
- A recognized social work qualification (e.g., BA/MA in Social Work).
- Current registration with Social Work England.
