An experienced Adult Social Worker is required to join a busy hospital-based team supporting adults with a range of complex health and social care needs. Working in a fast-paced environment, you will complete comprehensive assessments, identify eligible needs, and develop person-centred care and support plans that promote independence, dignity, and safe discharge from hospital. You will work closely with patients, families, carers, health professionals, community services, and care providers to ensure that decisions are well coordinated and focused on the individual’s goals.

Your responsibilities will include undertaking strengths-based assessments, reviewing existing support arrangements, arranging appropriate packages of care, and contributing to multidisciplinary discharge planning. You will manage a varied caseload, maintain accurate and timely records, prepare reports, and make informed decisions in line with relevant legislation, safeguarding procedures, local policies, and professional standards. The role may also involve completing mental capacity assessments, contributing to best-interest decisions, responding to safeguarding concerns, and supporting people who may have complex physical, mental health, cognitive, or social care needs.

Applicants should hold a recognised Social Work qualification and current registration with Social Work England, with demonstrable experience in adult social care, hospital discharge, or a related setting. You will need sound knowledge of the Care Act, Mental Capacity Act, safeguarding practice, and strengths-based approaches, together with the confidence to work collaboratively in a multidisciplinary team. Excellent communication, organisation, assessment, and record-keeping skills are essential, as is the ability to prioritise effectively under pressure. Experience working within a hospital team in or around Hackney would be advantageous. The successful candidate will be committed to promoting inclusion, choice, and positive outcomes while delivering a responsive, professional, and compassionate service.
