We are seeking a qualified Adult's Social Worker to join our Access & Duty Team. In this role, you will be at the forefront of assessing and addressing the needs of vulnerable adults, ensuring they receive the necessary support and services to enhance their well-being and quality of life.
Your key responsibilities will include:
Conducting comprehensive assessments of adults in need of social services. Developing and implementing care plans in collaboration with clients and their families. Providing information, advice, and support to clients regarding available services. Working closely with multidisciplinary teams to coordinate care and support. Maintaining accurate and up-to-date case records and documentation. Participating in safeguarding investigations and ensuring compliance with legal and policy frameworks.The ideal candidate will possess the following essential qualifications and skills:
A recognized social work qualification (e.g., BA/MA in Social Work). Registration with Social Work England. Proven experience in adult social work, particularly in assessment and care planning. Strong communication and interpersonal skills to engage effectively with clients and stakeholders. Ability to work independently and as part of a team in a fast-paced environment.
