Caritas Recruitment is currently recruiting for an experienced Social Worker to join the Preparing for Adulthood Team (PfA) at the London Borough of Bromley.

This rewarding role sits within Bromley's 0-25 Service and focuses on supporting young people aged 17½ to 25 with learning disabilities, autism and additional needs as they transition into adulthood.

Key Responsibilities

  • Undertake Care Act assessments, reviews and support planning.
  • Complete Mental Capacity Act assessments and Best Interests decisions.
  • Manage a caseload of young people transitioning into adult services.
  • Carry out safeguarding enquiries and risk management activity.
  • Work closely with families, health professionals, education providers and partner agencies.
  • Participate in Court of Protection and Best Interests work where required.
  • Maintain accurate assessments, reports and case records.

About You

  • Qualified Social Worker registered with Social Work England.
  • Strong Adult Social Care experience.
  • Sound knowledge of the Care Act 2014 and Mental Capacity Act 2005.
  • Experience undertaking assessments, support planning and safeguarding work.
  • Ability to work effectively within a multi-agency environment.

Desirable Experience

  • Preparing for Adulthood or transition work.
  • Supporting young adults with learning disabilities and/or autism.
  • Court of Protection experience.
